2360 East Pershing Blvd.
Cheyenne, WY 82001

The Release of Information office is open during normal business hours 8 a.m. – 4:30 p.m., located in the lower level of the hospital in Room C1-3.

A consent form may be mailed to you in the event you are here after normal working hours or you may pick up a consent form from the emergency room. Forms may also be downloaded from the choices at the right.
