Care Coordination/Home Telehealth: Health Hero Program
What Is It?
The Care Coordination/Home Telehealth is a program that uses an electronic monitoring system in the home that allows the VA to monitor your health status related to specific diseases.
The unit will ask you preprogrammed questions regarding your specific health issue. It will transmit your answers later through a toll free number to an internet file accessible to VA staff to monitor the next day. By monitoring responses, we look for a potential problem in your medical/mental health status. We will then intervene before it becomes a crisis that could require an Emergency Room visit or hospitalization. If we see a trend that is leading to a crisis, we will call you and try to alleviate it by advising you, educating you or contacting your Primary Care Provider for you.
Who Is Eligible?

Veterans who have been hospitalized in VA hospitals in the past year

Veterans who find they need to use outpatient services frequently

Veterans who live far from the VA Center or Clinics

Veterans who have a diagnosis pertinent to current established monitoring programs.

Veterans or veteran’s caregivers who can read the screen of the Health Buddy and can understand the technology of the program (education will be provided.)

Veterans who will be compliant and honest with responses.

Veterans who meet the program criteria.
