The Cheyenne VA Region Home Telehealth Team provides ongoing assessment, monitoring and case management of Veterans in their home. The program assists providers to provide “just in time” care for Veterans. |

How does Home Telehealth work?The Veteran receives free monitoring equipment in their home that allows the Home Telehealth Care Coordinator to monitor their vitals signs, provide education, and review symptoms the Veteran may be having and relay this information to their PACT team or Mental Health provider. The monitoring is done on a daily basis- in between a Veteran’s regularly scheduled PCP or Mental Health visits. This allows effective communication of the Veteran’s needs to their health care team when it is needed- not waiting until their next scheduled appointment. |
How is the health information sent to the Home Telehealth Care Coordinator?The veteran receives a small tele-response device for their home and all the equipment used to monitor their health data. The veteran may choose to send their data via their cell phone or laptop computer as well. Many of the tele-response devices use cell phone service to send the data securely to the Home Telehealth Care Coordinator. |

How Can you Enroll in the Home Telehealth Program?You can ask your Pact Team, PCP, Dietician, or Mental Health Provider for a consult to the program or contact any member of the Home Telehealth Team
