Mental Health Intensive Case Management

Mental Health Intensive Case Management (MHICM) Rural Access Network for Growth Enhancement (RANGE) Cheyenne Veterans Affairs Medical Center (VAMC)

The program provides Mental Health Intensive Case Management Services to veterans with a serious mental illness.  The MHICM RANGE program provides individualized Case Management services to Veterans and their families to strengthen their placements in a community setting and to decrease the occurrence of re-hospitalization.  The program currently serves Veterans in the Cheyenne and Laramie, Wyo., areas.  Veterans are seen two to three times per week. The team utilizes Motivational Interviewing and Social Skill Building groups within a Psychosocial Rehabilitation model.  The staff utilize the Assertive Community Treatment model to set up and monitor medication regimens and attendance and participation in medical and mental health appointments.  Since we have no inpatient psychiatric unit at the Cheyenne VAMC, referrals can be made to the MHICM RANGE program by outpatient medical and mental health providers via consults to the MHICM Program. 

Enhanced MHICM RANGE of Northern Colorado

The program provides Mental Health Intensive Case Management Services to Veterans with a serious mental illness.  The Enhanced MHICM RANGE program provides individualized Case Management services to Veterans and their families to strengthen their placements in a community setting and to decrease the occurrence of re-hospitalization.  This program also works with Veterans who are homeless or likely to become homeless in addition to their serious mental illness.  The program currently serves Veterans in the Northern Colorado area including (Fort Collins, Greeley, Berthoud, Windsor and Wellington).  Veterans are seen two to three times per week.  The team utilizes Motivational Interviewing and Social Skill Building mostly in individual settings.  The staff utilize the Assertive Community Treatment model to set up and monitor medication regimens and attendance and participation in medical and mental health appointments.  Since we have no inpatient psychiatric unit at the Cheyenne VAMC, referrals can be made to the Enhanced MHICM RANGE program by outpatient medical and mental health providers via consults to the MHICM Program.
